Fees

 We currently accept the following forms of insurance:

  • Blue Cross Blue Shield
  •  Medicaid
  • Presbyterian
  • Molina
  • United Health Care
  • Government Employees Health Association (GEHA)

We also take payment for services based on a sliding scale according to income (please be prepared to show verification of income on your intitial visit)

*No Client will be turned away due to inability to pay.

 

Cancellation Policy:

Our goal at Amistad is to provide quality service in a timely manner.  In order to value each client’s time with us, we have had to implement a “NO SHOW/LATE CANCELLATION” policy. 

We understand that circumstances may result in you needing to reschedule your appointment, however, we ask that in such case you give us a 24 notice by calling 575-523-2288. Failure to do so will result in a $25.00 charge to you, that is not covered by your insurance.  If you incur a charge of this kind, therapy services will not resume until the charge is paid. Repeated missed appointments and/or unpaid fees may result in being placed at the bottom of our wait list. We appreciate your cooperation!
